Registering with the Practice

Regular NHS Patients

If you live within the Practice Area then you are free to register here.  A telephone call to the Surgery will inform you whether you live within the Practice Area.

Please bring along your NHS Card, if you have one, and any details of past medical treatment.  Registration forms and questionnaires can be printed off below.

An appointment will be made with the Practice Nurse who will do a basic health check and will advise you as to further care that you may need.

Temporary Residents

If you are staying in the area for 3 months or less and your own GP is too far away to visit, you can register here as a Temporary Resident.  You will need to complete a form, entering your home address, your temporary address, and the name and address of your own GP.

Overseas Visitors

1. If you are a visitor from overseas or a British person who is living abroad, and paying your taxes abroad and you are NOT in this country work or to study, then we can give you treatment for emergencies only on the NHS.  Any treatment for long-standing illnesses or non-urgent conditions would need to be dealt with as a Private Patient.

2. If you are in this country to work or to study, then we need to see your Passport, Visa or other document such as an Identity Card or European Health Insurance Card before we can register you as an NHS Patient.  We will make a copy of the documents which we will send to Essex Health Authority who will arrange for you to receive an NHS Number.

Please note that if you cannot prove that you are in this country to work or to study, then you will have to be registered as a private patient.
